Upon the day of our session there are three forms to fill out. These forms help protect you and me! The forms are a service acknowledgement, release form, and an order form. These forms help clarify any questions or confusion of the released images as well as a guarantee that I Am Completing the service. these forms, once signed, are copied and emailed to you.
